The short answer: the Wegovy pill and Foundayo are the only two GLP-1 pills the FDA has approved for weight loss, and the biggest everyday difference is the routine. The Wegovy pill has to be taken first thing in the morning on an empty stomach with a little water, then nothing else for 30 minutes. Foundayo can be taken any time of day, with or without food.
They're also different kinds of drugs. The Wegovy pill is semaglutide, the same peptide as the Wegovy shot, made swallowable with an absorption helper. Foundayo is orforglipron, a small-molecule drug that isn't a peptide at all. In their main trials, the Wegovy pill's top dose averaged a bit more weight loss, but the trials were run separately and have never gone head to head in people with obesity. Self-pay prices are almost the same.
Status as of October 24, 2026: both pills are FDA-approved prescription drugs for adults with obesity, or overweight with a weight-related health problem. Both are covered by Medicare's temporary GLP-1 Bridge program at $50 a month for people who qualify. This post describes what the drug labels and trials say. It isn't medical advice. Ask your doctor which, if either, fits you.

How do the Wegovy pill and Foundayo work?
Both act on the GLP-1 receptor, the same target as the weekly GLP-1 shots. GLP-1 is a natural hormone that helps regulate appetite and how much you eat.
The Wegovy label describes semaglutide as a copy of human GLP-1 with 94% of the same sequence that "selectively binds to and activates the GLP-1 receptor" (Wegovy label). The Foundayo label says it "decreases food intake," likely through lower appetite, and that it "delays gastric emptying," an effect that's largest after the first dose and fades over time (Foundayo label).
The difference is what the molecule is. That's why the rules for taking them are so different.
How do you take the Wegovy pill? (and what about coffee?)
The label is strict. Take one tablet once a day "on an empty stomach in the morning with water (up to 4 ounces)," and don't take it "with other liquids besides water." Swallow it whole. Then "wait at least 30 minutes before eating food, drinking beverages or taking other oral medications" (Wegovy label).
So, after coffee? The label doesn't name coffee, but coffee is a beverage. Read plainly, the label's order is tablet first with plain water, then wait 30 minutes before coffee, breakfast or other pills. After eating? The label says empty stomach in the morning, so the tablet comes before food, not after.
If a dose is missed, the label says to skip it and take the next one the following day.
The reason for all this is absorption. Semaglutide is a peptide, and the gut breaks peptides down. Novo's own scientists wrote that oral peptides are "hindered by poor absorption across the gastrointestinal barrier and extensive degradation by proteolytic enzymes" (Sci Transl Med, 2018). The SNAC in the tablet helps semaglutide get absorbed in the stomach. Even so, the label puts absorption at "approximately 1% to 2%." Food, extra liquid or other pills in the stomach get in the way.
How do you take Foundayo?
The label is short: "Take FOUNDAYO orally once daily, with or without food." Swallow it whole, and don't take more than one a day (Foundayo label).
There's no water limit and no waiting period. The label reports "No clinically relevant food effect" on how much of the drug gets into the blood, and an absorption figure of 77% at the starting dose. Because orforglipron is a small molecule, it doesn't need a helper to get through the gut.
If a dose is missed, the label says to take it as soon as possible without doubling up. If 7 or more doses in a row are missed, it says the dose should be restarted lower.
One rule matters for women still using the birth control pill. Because Foundayo slows stomach emptying, the label advises switching to a non-oral method, or adding a barrier method, "for 30 days after initiation with FOUNDAYO and for 30 days after each dose escalation."
How much weight do people lose on each pill?
In their main trials, about 14% on the Wegovy pill's top dose and about 11% on Foundayo's top dose, compared with about 2% on placebo. Those are averages from two separate trials, so they aren't a true head-to-head result.
Wegovy pill. In the trial on its label (OASIS 4), 307 adults with obesity, or overweight with a weight-related condition, and without diabetes, took the 25 mg tablet or placebo for 64 weeks. Average weight change was -13.6% on the pill vs -2.4% on placebo. About 60% on the pill lost at least 10% of their weight, vs 14% on placebo (Wegovy label).
Foundayo. In ATTAIN-1, 3,127 adults with obesity, or overweight with a weight-related condition, and without diabetes, took Foundayo or placebo for 72 weeks. Average weight change was -11.1% on the top dose (17.2 mg) vs -2.1% on placebo. The middle doses averaged -7.4% and -8.3%. About 55% on the top dose lost at least 10%, vs 13% on placebo (Foundayo label).
Everyone in both trials also got diet and exercise counseling.
Wegovy pill vs Foundayo: has anyone compared them directly?
Not in people with obesity, and not at the Wegovy pill's weight-loss dose. A search of ClinicalTrials.gov on October 24, 2026 found no such trial.
The closest is ACHIEVE-3, a 52-week, open-label trial of 1,698 adults with type 2 diabetes. It compared orforglipron with oral semaglutide at lower, diabetes doses (7 mg and 14 mg), not the 25 mg Wegovy pill. Average weight loss was 9.2% on the top orforglipron dose vs 5.3% on oral semaglutide 14 mg, measured as if everyone stayed on the drug. More people quit orforglipron because of side effects: 9.7% vs 4.9% (Lilly, Feb 26, 2026). Lilly ran the trial.
So when you see "orforglipron vs oral Wegovy" posts, check which semaglutide dose they mean. The only direct comparison used the diabetes doses.
Wegovy pill vs injection: what changes?
Same drug, different delivery. The Wegovy shot is once a week. The pill is once a day with the empty-stomach routine.
In its main trial, the 2.4 mg weekly shot averaged -14.9% vs -2.4% on placebo at 68 weeks in 1,961 adults without diabetes (Wegovy label). That's close to the pill's -13.6%, but again from a different trial. The label also lays out how to switch between the two, which is your doctor's call.
What are the side effects of Foundayo and the Wegovy pill?
Mostly stomach and gut effects, as with every GLP-1. Nausea, diarrhea, constipation and vomiting top both lists.
On Foundayo's top dose, across both of its trials, the label lists nausea in 35% vs 10% on placebo, constipation 24% vs 9%, diarrhea 25% vs 11%, and vomiting 24% vs 4%. About 10% on the top dose stopped because of side effects, vs 3% on placebo (Foundayo label).
For the Wegovy pill, the label says the side effects in its trial were similar in type and frequency to the Wegovy shot's. In that trial, 6.9% on the pill stopped because of side effects, vs 5.9% on placebo (Wegovy label).
Don't line those numbers up as a scoreboard. The Foundayo label itself says side-effect rates from one drug's trials "cannot be directly compared to rates in the clinical trials of another drug."
A few label points matter more after 45:
Both labels carry a boxed warning about thyroid C-cell tumors seen in rodents.
Thyroid medicine. In a study with the semaglutide tablet, exposure to levothyroxine rose 33%. The Wegovy label says to monitor the effects of other pills taken alongside it.
Other pills on Foundayo. The Foundayo label flags interactions with some common drugs. Taking it with simvastatin doubled exposure to that statin's active form.
Few older adults were studied. 13% of Foundayo-treated patients in its trials were 65 or older. In the Wegovy pill trial it was about 10%.
Your pharmacist can check your current pill list against either label.
Wegovy pill side effects in women, and is there menopause data?
There's some sex-specific data on the labels, and each company has published one menopause analysis. None of it is a dedicated trial in menopausal women.
Hair loss. On Foundayo, hair loss was reported in 7% of women vs 0.9% of men (3% vs 0.7% on placebo), and the label links it to weight loss (Foundayo label). The Wegovy label gives a sex split only for the shot: 4% of women vs 0.9% of men on 2.4 mg (Wegovy label).
Bones. In Wegovy's large heart outcomes trial, which used the shot, hip and pelvis fractures were more common on Wegovy than placebo in women: 1% vs 0.2%. They were also more common in people 75 and older. The pill label doesn't report this separately.
Menopause. Neither label mentions menopause. The data comes from company analyses done after the trials ended:
Foundayo: a Lilly-sponsored post-hoc analysis of ATTAIN-1 and ATTAIN-2 found women on the top dose lost significantly more weight than placebo whether they were pre-, peri- or postmenopausal (Lilly). Reported losses were up to about 14% and similar across the three stages (Medical Xpress, Sept 26, 2026).
Wegovy pill: a Novo post-hoc analysis pooled the pill trial with the shot trials. Postmenopausal women lost an average of 15.7% over 64 weeks, vs 18.2% for premenopausal women (Novo Nordisk, Nov 5, 2025). Because it mixes pill and shot data, it isn't a pill-only result. Novo calls such analyses "hypothesis-generating."
I couldn't find any published data on side effects by menopausal stage, or on using either pill alongside hormone therapy. If that matters to you, it's a question for your doctor.
How much does the Wegovy pill cost? How much does Foundayo cost without insurance?
Self-pay, both run $149 to $299 a month. Here's every price as listed on October 24, 2026.
Dose step | Wegovy pill (NovoCare) | Foundayo (LillyDirect) |
|---|---|---|
Starting dose | $149 (1.5 mg) | $149 (0.8 mg) |
Second step | $199 (4 mg) | $149 (2.5 mg), reduced through Dec 31, 2026 |
Third step | $299 (9 mg) | $199 (5.5 mg), reduced through Dec 31, 2026 |
Higher doses | $299 (25 mg) | $299 (9 mg). $299 (14.5 and 17.2 mg) if refilled within 45 days |
"A month" means | 30 tablets | 30 days |
Sources: NovoCare, LillyDirect, both read October 24, 2026.
The catches: NovoCare labels its pill prices a "limited time offer" without an end date. Lilly's two lower prices end December 31, 2026, and its top two doses go to a "regular price" if a refill is more than 45 days late. Lilly's page doesn't say what that regular price is. Both companies say they can change or cancel these programs at any time. With commercial insurance that covers it, Lilly says Foundayo can cost as little as $25 a month.
Is the Wegovy pill or Foundayo covered by Medicare?
Yes, through a temporary program. The Medicare GLP-1 Bridge covers Foundayo, the Wegovy pill and the Wegovy shot for $50 a month, "no matter your income level," from July 1, 2026 through December 31, 2027 (Medicare fact sheet, CMS).
To qualify, Medicare's fact sheet lists four requirements:
You have Part D drug coverage, through a standalone drug plan or a Medicare plan that includes drugs.
You aren't already getting a GLP-1 through your drug plan.
You don't have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease. People with those conditions should ask their drug plan, which may already cover a GLP-1.
You're 18 or older and have a BMI of 35 or more, or a BMI of 30 or more with certain heart failure, hard-to-control blood pressure or kidney disease, or a BMI of 27 or more with prediabetes or a past heart attack, stroke or blocked leg or arm arteries.
Your doctor sends the prescription and a form for Medicare approval. The $50 doesn't count toward your Part D deductible or yearly out-of-pocket limit. What comes after 2027 isn't settled: CMS says a longer-term program called BALANCE won't launch in 2027.
Foundayo after Zepbound or Wegovy shots: can it keep the weight off?
One trial tested exactly that. In ATTAIN-MAINTAIN, 376 people who had already spent 72 weeks on Zepbound or Wegovy shots, and whose weight had leveled off, were switched to orforglipron or placebo for a year (Lilly, Dec 18, 2025).
People who came off Wegovy kept all but 0.9 kg (about 2 pounds) of their earlier loss on average. People who came off Zepbound kept all but 5.0 kg (about 11 pounds). Put another way, the Zepbound group kept about 75% of its earlier loss on orforglipron vs 49% on placebo, and the Wegovy group about 79% vs 38% (Weill Cornell Medicine, May 12, 2026, on the Nature Medicine paper). Lilly sponsored the trial.
That's a trial result in people who had already reached a plateau. It isn't a switching plan. Whether and how to switch is a conversation for your doctor.
Is Foundayo approved in the UK or Europe?
In the UK, yes. The MHRA authorised it on August 10, 2026, but it "is not available currently via the NHS" while NICE reviews it (GOV.UK, Aug 10, 2026). In the EU, the EMA recommended approval on October 16, 2026, and the final decision now sits with the European Commission (EMA, Oct 16, 2026).
So which one is better?
Neither, on the evidence we have. They've never been tested against each other for weight loss.
What the labels do show is a trade-off. The Wegovy pill's top dose averaged more weight loss in its trial, it's also approved to lower heart attack and stroke risk in people with heart disease, and it's the same drug as a shot with years of data. Foundayo is easier to fit into a day, with no empty-stomach rule and no 30-minute wait, which can matter if you already take morning pills. Prices are nearly the same, and Medicare's Bridge treats them the same.
The pill you'll actually take every day, that your doctor agrees fits your health history, is the one that counts. If you want to watch what these drugs do to your numbers, a baseline panel helps. Here's the minimum bloodwork stack. For why GLP-1s changed the weight-loss conversation in the first place, see GLP-1s exposed the real fat-loss problem. And for what's coming next, there's the next-gen pill pipeline.
